What Is High and Tight Circumcision?

High and tight circumcision is a description of how the foreskin is altered according to the patient’s specifications. Circumcision is not a “one size fits all” procedure, so Dr. Bidair is careful to honor his patients’ wishes and deliver the best results in Southern California. 

Styles of Circumcision

In the course of a circumcision, the foreskin is not simply removed; it is skillfully repositioned and reduced. To create the best results, Dr. Bidair considers the final outcome. He can alter the foreskin in one of four ways: low and loose, low and tight, high and loose, or high and tight. The dimensions refer to how the foreskin will sit against the shaft of the penis after the circumcision surgery is complete.

With a high and tight circumcision, for example, the foreskin is retracted until its interior wall is now facing the exterior, cradling the shaft supportively.[1] The surgeon removes tissue that was previously along the exterior of the foreskin and then carefully sutures the incision to preserve the contours of this “high and tight” acclimation. The subtle scar will be positioned high up the shaft when the penis is flaccid, and the feel of the final result will be secure and supportive.

Preparation and Procedure

During your consultation, the staff at California Adult Circumcision will provide you with detailed instructions to prepare you for surgery. These directions will be specific to your health and anatomy, so please follow them closely.

To accomplish the high, tight look and feel, Dr. Bidair must consider the dimensions of the patient’s foreskin. The way the prepuce (foreskin) aligns with the glans (penile head) will inform the doctor which surgical approach to pursue.

Dorsal Slit Technique

If the foreskin sits tight against the penis, it may require a dorsal slit method of circumcision. The procedure begins with the appropriate anesthesia to keep you safe and comfortable. From there, Dr. Bidair gently makes an incision perpendicular to the outer ridge of the foreskin. This allows him to retract the prepuce back to its ideal position. Dr. Bidair can then create a “high and tight” look and feel by securing the inner lining of the foreskin securely against the lower shaft of the penis.

Sleeve Method

When the foreskin hangs loose away from the penis, a sleeve technique may be the optimal approach. Dr. Bidair skillfully removes prepuce tissue along the circumference of the foreskin, like a tailor hems a sleeve (hence the name). The doctor then closes the incision to create the contours specified during the patient’s consultation. 

Proximal Circumcision

You can enjoy the benefits of circumcision while leaving the foreskin intact. Proximal circumcision removes skin along the base of the penis and pulls the material along the shaft back to cover the incision. This, in turn, retracts the foreskin until the head of the penis is exposed. The base is then sutured gently, allowing the new contours of the penis to recuperate. 

To achieve a high and tight reconfiguration, Dr. Bidair will most likely opt for either a dorsal slit approach or the sleeve technique. The exact nature of your circumcision is ultimately your decision, and Dr. Bidair will explain all of your options during a comprehensive consultation. 

Healthy Recovery

Immediately following your procedure, a trained medical professional will monitor your progress until you are ready to resume the recovery process from the comfort of your own home. Please assign a caregiver to drive you to and from the clinic, as you will not be allowed to operate a motor vehicle after surgery.

Refrain from strenuous activities for the first 1-2 weeks of your recovery and avoid sexual intercourse for at least 2-4 weeks. Great results are worth the wait, so please do not rush the healing process.
