The diagnosis of pearly penile papules is fairly common for men of all ages. Pearly penile papules are small, dome-shaped bumps that appear around the head of the penis, and they get their name because they resemble small pearls. They are completely normal and are considered a common condition among men. (1) However, while they are a normal and benign condition, they can be a source of distress among many men, both in their daily lives and during intimacy. Pearly penile papules are not indicative of sexually transmitted diseases or infections, poor hygiene, or other issues; nonetheless, many men will seek treatment or removal if they do not disappear on their own.

About Pearly Penile Papules
Pearly penile papules are benign growths that typically appear as tiny white or flesh-colored bumps around the corona, the ridge at the base of the glans (head of the penis). They are not harmful and do not pose any medical risks; however, many men prefer to seek treatment for cosmetic reasons, as they may cause anxiety or embarrassment when engaging in intimate relationships. (2)
The presence of pearly penile papules is a natural phenomenon and usually appears in men aged 20 to 30 years. Statistics indicate that approximately 14-48% of males may experience this condition, with the majority being uncircumcised. (1) While nothing can be done to prevent them from occurring, various treatment options exist should one desire removal.
Causes of Pearly Penile Papules
The most common cause of pearly penile papules is standard anatomical variation. Rather than being a genetic predisposition, some men are simply born with or develop pearly penile papules naturally. Fortunately, the development of pearly penile papules is harmless. Their presence is not related to sexual transmission or poor hygiene, and they do not cause any physical discomfort, interfere with intercourse, or contribute to other penile conditions like urinary burning. However, as stated, despite being benign, they are still a source of embarrassment for many men, especially when they persist and do not fade on their own.

Preparation
Preparing for the procedure is straightforward. You will receive specific preoperative instructions, which may include:
- Discontinuing blood-thinning medications.
- Avoiding alcohol consumption and smoking in the days leading up to your appointment (if applicable).
- Arranging transportation to and from the clinic to minimize stress on the day of treatment. However, you can easily drive yourself.
Ensuring these guidelines are followed closely will help promote optimal results.
Treatment and/or Removal Options
Pearly penile papule removal procedures are performed in our clinic and do not take long. In most cases, Dr. Bidair will remove your papules with electrocautery. We will administer a local anesthetic to enhance your comfort throughout the procedure.
There are two other main pearly penile papule treatment/removal options available for patients. After assessing your condition, Dr. Bidair will determine what is best for your needs. Even though cryotherapy has been used to treat the papules, this treatment is not used by Dr. Bidair.
Another method is laser therapy. (1) Dr. Bidair may employ an ablative CO2 laser to remove the papules if your case requires it. In these treatments, you will receive a local anesthetic on your penis and also be provided with protective eyewear. He will carefully maneuver the laser over each papule, and the heat emitted by the laser will cause the cells inside to lose their viability, leading to the papules falling off. (2) Non-ablative, fractional, and pulsed dye lasers have also been shown to be effective at treating pearly penile papules. (2)
Recovery
Recovery from treatment is minimal, with many patients returning to normal activities the same day as the procedure. Initial mild discomfort can be managed with over-the-counter pain medications. Swelling and redness may persist but generally resolve quickly. It is crucial to refrain from activities that may irritate the treatment area for about one week and to keep the entire genital area clean during this time.
Results
After completing the procedure, patients can expect visible improvements within several weeks. As healing progresses, the treated area takes on a smoother appearance, contributing to enhanced self-esteem and comfort. Results tend to be long-lasting, as new papules do not usually develop after treatment. Some people may need more than one treatment session.
Corresponding Procedures
Pearly penile papules are sometimes misdiagnosed as genital warts or sebaceous cysts (or vice versa). If you have already been diagnosed with these or if Dr. Bidair identifies these as your condition rather than pearly penile papules during your consultation, you will still have treatment options available at our clinic. Cyst removal is a minor surgical procedure. After applying a local anesthetic, Dr. Bidair will make an incision and remove the cyst. Patients can return home immediately after. Depending on the nature of your genital warts, you may receive a prescription or undergo treatments similar to pearly penile papule removal.
