Thinking About Revision Circumcision?

Circumcision is the surgical process of removing the foreskin around the head of the penis. It has been practiced for thousands of years and varies in popularity and methods across cultures. Many men (approximately 80% in the United States and 40% globally) will receive the procedure within the first few days of their lives. (1) However, like all surgical procedures, there is some risk that revision will be necessary. If your circumcision has resulted in an undesirable penile appearance or causes issues with urination, sex, exercise, or other activities, then a revision circumcision may be beneficial to your health, comfort, and confidence.

Dr. Mo Bidair is passionate about urological health and understands its significance in prolonging the lives of men. His extensive background in adult circumcision makes him stand out as one of the most uniquely qualified doctors in the country for revision circumcision procedures. Patients regularly experience quick, painless procedures with minimal recovery times and effective results.

When is Revision Circumcision Necessary?

A botched circumcision refers to when the surgical procedure of removing the foreskin from the penis is not performed correctly. It can happen for various reasons, leading to complications such as:

  • Excessive Skin Removal: This occurs when too much foreskin is removed, leaving insufficient coverage over the glans (the head of the penis).
  • Under-Circumcision: This happens when not enough foreskin is removed, leading to continued coverage of the glans, which may cause hygiene issues.
  • Scarring: Improper healing can lead to excessive scarring, causing painful erections and discomfort.
  • Displacement of the Scar: Scars can form in unacceptable locations, impacting sensations or leading to pain during intercourse.
  • Meatal Stenosis: Narrowing of the urinary opening due to scarring can create pain during urination.
  • Phimosis: Tightening of the remaining skin can impair functionality.
  • Ambiguous Appearance: A botched circumcision can result in an appearance that does not meet expectations or social standards.

Poorly performed circumcision raises several concerns, both medical and emotional. For some individuals, these issues can weigh heavily on self-esteem, body image, and comfort during sexual activities. Some people may have had a botched circumcision due to a lack of experience or techniques used by practitioners in pediatric urology. Others might have experienced complications from wounds getting infected, especially if proper post-operative care was not followed.

Benefits of Circumcision

Circumcision offers many important benefits to men. Enhancing these procedures and correcting past mistakes with a revision circumcision can not only improve these benefits but ensure that they last much longer too. Just some of these include:

  • Enhanced, ideal appearance: Any awkward scarring or blemishes that were formed as a complication of a previous circumcision will be eliminated, giving your penis an appearance you can be proud of and confident in.
  • Improved hygiene: Dirt, debris, and body oils can become trapped under the foreskin and this can be difficult to properly clean. Revising a circumcision can make it easier to clean your penis and maintain proper hygiene.
  • Reduced risk of penile cancer and HIV: the World Health Organization has found that proper circumcision can significantly reduce the risks of cancer and HIV infections in men. (1)
  • Eliminated pain or discomfort: a botched circumcision can create pain during sex or activities, discomfort in certain clothing, and potentially even make urination uncomfortable.

Candidates

Any patient who is experiencing complications from an unsatisfactory circumcision or who is simply not content with the appearance of their penis after their circumcision can qualify for a revision circumcision. If you want to proceed with planning this procedure or think you may benefit from a revision circumcision, please do not hesitate to schedule a consultation with Dr. Bidair. This is the best way to discuss your options and understand what revision measures are available for your circumstances.

Personal Consultation

When you meet with Dr. Bidair for your consultation, you will first discuss your previous circumcision and any past revisions. If you received your circumcision as an infant, we do not expect you to have the full details. You will discuss your motivations behind wanting to revise your circumcision results. You will also discuss the complications you are facing as a result; both the mental and physical ones. Dr. Bidair will also review your medical history to screen for any potential risks, allergic reactions, or other complications that may occur during your revision circumcision. Once he has collected all of the necessary information, he will create a procedural plan that is specific to your case and ensures optimal results.

Preparation

Revision circumcisions are minimally invasive procedures performed with a local anesthetic, so very little preparation is required. You will not have to fast, alter your diet, or adjust your medications before your procedure. However, we still recommend taking a few beneficial preparations to reduce stress and maximize comfort following your procedure.

Dr. Bidair will give you a prescription for pain medications at your consultation or another preparatory appointment.

Procedure

Your specific revision circumcision procedure will depend on your individual circumstances. Typically, more foreskin, with possible scars, irregular cut lines or suture marks, is removed to create a neater, more attractive appearance and improve function. This also helps to reduce the risk of phimosis, a condition where the foreskin has trouble returning to its original position once pulled behind the head of the penis.

Revision circumcisions employ techniques similar to standard circumcisions. All circumcision procedures take special caution to minimize bleeding and preserve sensation in the head of the penis.

Recovery

It is important to take care of the treatment area after your procedure to minimize the risks of infection or any other complications. The groin and genital areas are prone to excessive sweating, so we recommend washing your incisions at least twice per day. Normal body wash or soap can be used, but most patients find that hand soap or facial cleansers are much more pleasant and gentle on sensitive skin. All patients must change their dressings as necessary. These dressings will typically cover the entire penis shaft and have additional gauze or cotton above the incision sites. (1) Dr. Bidair recommends using gauze and bandages when applying new dressings. Any Band-Aids or adhesive dressings can cause discomfort when they are removed from the penis. Our patients typically experience a healing time of 4 weeks. (1) However, there is still healing and remodeling that takes place for 3 to 6 months before the final result. Some men heal faster and others heal slower. It is important to avoid any form of sexual activity for at least 3 weeks.

Results

Patients who receive a revision circumcision are regularly satisfied with their results. (2) Once your condition has improved and you see a significant advancement in your healing, you will immediately notice an improvement in your foreskin. Any botched results, jagged lines, or scar tissue that were previously visible on your penis will be eliminated and you will be left with an attractive penis that matches your ideal goals. You will also experience less pain and discomfort in activity, clothing, sex, and urination.

Corresponding & Complementary Procedures

There are various options available to enhance the appearance of your penis and scrotum in addition to a revision circumcision. If there are other features of your penis that you would like to alter or enhance, Dr. Bidair can help you achieve your goals.

Scrotoplasty

A scrotoplasty is a procedure that can help you enhance the appearance, hang, and tightness of your scrotum. Some men are not content with the appearance of their scrotum or the level to which it hangs. Receiving one of these procedures can increase your comfort and your confidence.

Scrotox

“Scrotox” is a portmanteau of “scrotum” and “Botox” and refers to the process of receiving botulinum toxin injections in the scrotum to enhance its appearance by reducing wrinkles and accentuating the size of the scrotum and testicles.

Pearly Penile Papule Removal

Pearly Penile Papules, also known as PPPs, hirsuitoid papillomas, or papillomatosis, is a common, benign condition in men. While they do not pose any threatening effects, they are still a frustrating feature on one’s penis. PPP removal is one of Dr. Bidair’s most commonly performed procedures, and his experience can ensure satisfactory results.
