A scrotoplasty, or scrotal lift, is a surgical procedure that reshapes, tightens, and reduces the size of the scrotum by removing excess skin and tissue. Many men have webbing between the penis and the midscrotum, leading to a “sail-like” appearance of the excess skin and visually reducing the apparent length of the shaft on the back side of the penis. Many men seek this procedure to improve appearance, comfort, or function after aging, trauma, or disease creates changes in their scrotum anatomy. Scrotoplasty techniques are also common in gender-affirming surgeries, where surgeons create a scrotum from labia tissue.
California Adult Circumcision is led by Dr. Mo Bidair, who is board-certified by the American Board of Urology and an active staff member in three hospitals in addition to his full-time practice. Dr. Bidair’s in-depth understanding of urology and commitment to patient-centered care allow him to achieve the best possible outcomes. His techniques consistently create aesthetically pleasing results with minimal scarring.

About Scrotoplasty
A scrotoplasty procedure targets the skin pouch beneath the penis that holds and protects the testicles. This procedure reduces sagging, improves hygiene, enhances cosmetic appearance, and improves comfort during physical activities like running and bicycling.
Some individuals are born with a condition that affects their scrotum, while others can develop certain related conditions in adulthood. (1) Scrotoplasty can address a wide range of concerns, helping men live their lives with comfort and confidence.
Benefits
Scrotoplasty offers both cosmetic and functional advantages, including:
- Enhanced comfort during activity
- Aesthetic improvement
- Correction of webbed penis
- Improved hygiene
- Reconstruction of the scrotum
- Reduced discomfort in underwear
Candidates
If you are looking to correct sagging skin, raise your testicles, remove webbing between the penis and the midscrotum (“sail-like” appearance), or reconstruct the scrotum for functional relief or cosmetic improvement, then you may be a good candidate for this procedure. Common candidates for scrotoplasty are men with enlarged scrotums that cause discomfort and those with penoscrotal webbing. Candidates must also be in relatively good health, hold realistic expectations for their results, and be non-smokers or willing to quit before their procedure.
Why Should I Get a Scrotoplasty?
Scrotoplasty adjusts the appearance of the scrotal sac and improves function. This procedure aims to improve aesthetics, restore function, and alleviate discomfort.
Aesthetic Scrotoplasty
Men who seek aesthetic scrotoplasty often report having a scrotum that droops below the end of the penis by 1-2 cm. Dr. Bidair can remove excess skin and tighten remaining tissue to create a raised, more appealing look. (2)
Scrotal Webbing
Scrotal webbing is a condition where the skin of the scrotum is attached to the majority of the ventral penile shaft, which reduces genital function as well as the cosmetic look of the penis and scrotum. This creates a buried appearance and, in some cases, causes discomfort and sexual dysfunction. Dr. Bidair assists by severing the webbed skin in a Z-plasty pattern to release the tension and prevent the web from returning.
Sagging or Enlarged Scrotum
If you are experiencing a loose scrotum and sagging skin, a scrotoplasty can help tighten and reduce its size. This type of scrotoplasty is more common in older adults when they lose muscle tone. It can also make sexual activity, exercise, and other physical activities comfortable again for patients with enlarged scrotums.
Trauma, Injury, or Infection
Scrotoplasty can repair a scrotum after damage from trauma or injury. This can also treat a life-threatening bacterial skin infection called Fournier’s gangrene, which causes the skin around the penis and scrotum to die. The treatment for this infection requires multiple surgeries to repair the scrotum.
Personal Consultation

Lastly, he will make a personalized treatment plan and provide a cost outline for your procedure. His expertise and urology background help him create aesthetically pleasing, properly functioning results. By taking the time to meet with you and explore your concerns, he can help you achieve the same.

Preparation
Preparing for your scrotoplasty is crucial, as this ensures a smooth procedure and successful outcome. Dr. Bidair advises you to:
- Refrain from alcohol or nicotine consumption at least two weeks beforehand
- Pause blood-thinning medications and supplements
- Cleanse the area with antiseptic soap one to three days before your procedure
- Complete all required blood tests (if necessary)
- Arrange a ride back home if possible.
- Prepare items like gauze and tight underwear
Procedure
Dr. Bidair will begin your procedure by administering local anesthesia to your scrotum. The exact incision varies depending on the kind of scrotoplasty to be performed. The incision may be along the midline of the scrotum (the crease between the scrotum and the penis) to reconstruct or in the back of the scrotal sac to remove the excess skin for the desired results. After removing the excess skin, a tighter, lifted, and enhanced appearance is achieved. Dr. Bidair will then close the incisions, and he may place a drain in the scrotum to minimize swelling. He will also provide a support garment to stop your scrotum from shifting. The procedure typically takes 2 to 3 hours from start to finish.
Results
Shortly after your procedure, Dr. Bidair will monitor your health before releasing you home that same day. Dr. Bidair will also schedule the appropriate follow-up to check up on your recovery.
You will likely experience swelling and bruising, which are common and will subside within a couple of weeks. As you rest and recover, you must avoid heavy exercise for about six weeks. The final shape and results are typically visible within three months.
Scrotoplasty offers a tightened, lifted, and more aesthetically symmetrical scrotum. The procedure also reduces sagging skin, which can help alleviate pain, pinching, or discomfort. Patients often feel more comfortable and confident after their procedure.
